Give a loop diuretic a intravenously d (e.g. frusemide 40-80 mg)
  • Ask patients to remain in bed afterwards. b
  • For large doses, consider using an infusion d  

Why?

  • Patients who lie supine for 6 hours compared with walking about, have a larger diuresis (on average 500 ml more) and a larger natriuresis (on average 50 mmol more). b
  • Continuous i.v. frusemide causes a greater diuresis (on average 600ml/day) and natriuresis (on average 60 mmol/day) than bolus infusions in patients on high-doses a but has no clear effect on mortality. d
