The following features increase the risk of an intracranial bleed: a
  • supratherapeutic INR ( > 4.0)
  • previous stroke
  • prosthetic heart valves
  • old age 

Why?

Previous strokes and prosthetic heart valves increase the risk of intracranial bleeding
