Major bleeds
  • Stop LMWH a
  • Protamine sulphate (1 mg inhibits 100 IU of dalteparin). Maximum dose 50 mg by slow iv infusion. d
    • Note: fresh-frozen plasma does not reverse the anticoagulant effect of LMWH

Minor bleeds

  • Consider stopping LMWH  d

Heparin-induced thrombocytopenia

  • Stop LMWH. a
  • An alternative is danaparoid. d  

Other complications

  • subcutaneous haematoma at site of injection d
  • osteopenia in prolonged administration  c
